Name and Email are collected, but NOT viewable by the form creator. These fields are only used for sending confirmation emails so respondents can manage their responses.

Start on the Form Options page for your form.

Scroll down to Form Settings and 'Enable Incognito Mode'

When Incognito Mode is enabled, a message will show on the form that informs respondents that their name and email will be collected, but NOT VISIBLE to the form creator.
STRICT PRIVACY MODE
Names and emails are NOT collected at all.

On the Form Options page, select 'Enable Strict Privacy Mode'.

When Strict Privacy Mode is enabled, the Name and Email fields are disabled and not collected.
CREATOR RESPONSES VIEW

When you view Responses for a form with Incognito Mode or Strict Privacy Mode enabled, responses will be shown as "Anonymous".
